Transfer mode (Q15)

It is very common for entities to incorrectly set the transfer-mode field in octet 4 of Setup bearer capabilities to 01, which indicates "packet mode." However, 7.2.2.1/H.225.0v2 requires it to always be set to 00, for "circuit mode."
